Aren't the medical exam requests done before PPR?

It might be a visitor visa. If IRCC thinks a visitor visa is more appropriate, they will grant you that instead of a super visa. This could be why you haven't received the request, or they might ask you later. You will find out the type of visa you received once your passport has been stamped, indicating whether it's V-1 or PG-1.

Hi after passport stamp do we get any other letter like we get approval letter after work permit stamp
 
Not usually.
Also, perhaps a bit early to ask but just out of curiosity, would you know if one can apply for a super visa after parents have visited Canada once and returned to home country within 6 months?

Yes no issue. Would suggest they reestablish some ties and not apply as soon as they return.
