Statutory declaration of common-law union

Hi Folks,

I've seen tons of different topics regarding this form, but still couldn't find the answer.

1. The section "...declare that we have cohabited in a conjugal relationship...":
Should I use here the day when we started living together as the start date? right?

2. "Name of the person who administer the declaration".
Who should verify my declaration? Could it be a friend of mine? Or it has to be a authorized organisation?

Any thoughts on this?

1. Yes, you put how long you have been living together and also the day you started. So for instance it would be "... solemnly declare that we have cohabited in a conjugal relationship 2 year(s) from 2015-04-29 ..."
