They way I sent my application for my husband, lahat original. BIRTH CERT would have to be the one from NSO sis same with AOM. Just make sure to keep photocopies of the important documents just so you have them as well.
I also included yung medical receipt na nareceive nya sa St. Lukes nung nagpamedical sya, and then of course I made a copy of that.
I like to make copies of things that will be difficult to get just in case someone misplace them at the Immigration na meron pa din akong ma provide just in case and it's for my own sake to know na binigay ko sa kanila lahat ng proper documents.
With regards to the pictures, my husband and I, we had quite a bit of pictures because of our travels. So what I did, nilagay ko mga 6-8 pictures in one 8x10 photo paper (or you may use another type of paper i'm sure) and then I printed them as a lump sum. I also added captions on each page. ie. dates when it was taken and where it was taken and if there was an occasion and if we have friends in the pictures then I added their name as well, with their consent of course.
Oh and this is important, Option C from CRA will have to be the original as well. But make sure to make a copy of this document because it takes a couple of weeks to get them. It would have to be the most recent taxation year so that will be for 2012. If Notice of Assessment for 2012 is available, it wouldn't hurt to send the original as well and of course, I can't stress this enough, make sure to make copies of everything.
