Yes, the OWP app is sent with the inland PR app to Mississauga. CIC will forward it on to the appropriate office.
The medical is not required for an OWP unless she wants to work in certain occupations such as childcare.

This form is generic. For the requirements that don't apply, simply put N/A or write "OWP" in the field. Across the top of IMM5710, write in big letters something like "Open Work Permit app to be processed with inland spousal sponsorship app".

Where is your spouse from? It may be better to consider an outland application. Your spouse can remain in Canada extending her status as a visitor while the outland app processes.
