2. Schedule A – Background Declaration(IMM 5669)
3. Additional Family Information(5406)
4. Spouse/Partner Questionnaire(IMM 5490)
5. Supplementary Information Form – Pakistan
6. Residency questionnaire – Pakistan
7. Authorization to Disclose Information – Pakistan
8. Application to Sponsor, Sponsorship Agreement and Undertaking(IMM 1344)
9. Sponsor Questionnaire(IMM 5540)
10. Copy 2 of Receipt(IMM 5401)
11. Birth Certificate(Original) with Translation
12. Marriage Certificate – Nikk Nama (Original)
13. Canadian Citizenship Certificate for Sponsor(Copy)
14. Canadian passport pages showing all details.
15. Proof of Employment for Sponsor.
16. Record of Landing(IMM 1000)
17. Certified Copy of National Identity Card.
18. Original Educational Document for Sponsored Spouse
19. If you live outside Pakistan more then one year then must submit the police clearance certificate
19. Copies of Passports including expired passport
20. Wedding Photos
21. Wedding Card
22. Proof of Sponsor’s visits such as e-tickets, boarding passes, copies of sponsor’s passport showing entry/exit stamps.
23. Documentary evidence of Sponsor now supporting financially.
24. Original Police Certificates and Clearance.
25. Proof of Medical Examination
26. 9 Recent Photos according to photo Specification
27. Proof of relationship – copies of emails, online chats, Skype etc.
