We did one before submission. It expired while the case was on the shelf. We were asked to redo it, when CIC was ready for it, because it was already 3 months invalid. We ended up delayed a month because of that (3 weeks waiting for an appointment; 1 week additional time to process and attach to the file).
Oh! That's terrible. Spending twice on medical, and CIC suggests doing medical upfront to reduce the time of processing your application...

Not anymore actually, you can't do upfront now, you have to wait for them to request it.

Update:
On 2017-06-21 my husband received an email advising him that he was approved and eligible to sponsor myself and three kids. A few hours later I received a letter on CIC advising me that I was eligible for permanent residency and that they were requesting medical checks for myself and the kids. I also need consent letters from the other parents in the US giving permission for the children to immigrate to Canada. I have a timeline of 30 days to get the medical checks and the consent letters back into CIC. So while I am super super excited that finally this is moving forward, I am also super overwhelmed. The letter also stated that if I had not applied for my OWP I should do so now. Which is what brings me to the group....I have questions...anyone who has been following my situation knows that I applied for OWP back in March and it was denied because I was 4 days past the restoration period. So here we go again.....

The letter I received is this the first approval letter I have been waiting on?
When I go to CIC website and try to apply for OWP, it asks me all these questions to see if I am eligible. Because I am out of status, even though I have received a first approval letter (I think)...it tells me I am not eligible to work in Canada, and I must leave Canada. Not sure how to proceed. Even when I just pull the application for OWP it asks for status, and I am not sure what to put there. I have been trying to call IRCC all morning to no avail...I go through the long automation and then it says, "high call volume, please call again later and hangs up on me"

Help!

Was only $190. Wasn't terrible.

CONGRATS!

THAT'S SA AND AIP! That's first-stage approval, done! You can now apply for work or study permits (and if you need it still, health care). Depending on where you are in Canada (I forget now if you mentioned it), you might get DM and a landing quickly after medicals and the consent being filed. If you can get the consent before the 30 days, then you might want to just wait for landing and save the cash on the OWP. It will take 4 months to get it, so it may not really be worth it. You can definitely land without status.

You just have to get the medical started for that 30 day clock to stop. Also, will you be able to get consent from your former partner? If you will need more time, you can send them a CSE and prove to them that you've tried to get that consent (and you're waiting). That will stop the clock on that too.

Thanks so much! It's been a long time coming. But how do I fill out my status on my OWP? Also, is there a set price for the medical checks....or does it vary by place? I called one place and its going to cost 1366.00 for the four of us to have our medical checks!!!! Wasn't sure if this is just the set rate.

Look above -- I updated my posting.

It does vary. The doctors can charge whatever they want. In Kitchener, ON it's $190 per person (incl blood work and the chest x-ray).

Work permits are here: http://www.cic.gc.ca/english/information/applications/work.asp

Decide if you are applying online or in paper first... You are out of status (means you need to also request to restore status, and pay the $200 restoration fee).

We are in Alberta. I am not familiar with all of the immigration lingo...so help me out lol. I should be able to get the consent, I just worry that it won't come in time to get it back to CIC in 30 days. I suppose I should call several places and get price quotes on medicals to find out which is the best price. What is DM? Also, IRCC told me I could not land without a status. So if I wait on the OWP to save some extra cash, I will still be able to land? And what is a CSE? lol. I am new to this you know. :)

Maybe I should come to Ontario for my medical checks lol, jk. I would be applying for the permit on paper most likely. I for whatever reason cannot get myCIC to accept the scanned applications.

Sorry.... I also updated the last post (again, look above).

DM - Decision Made
CSE - Case Specific Enquiry (IRCC Web form: https://secure.cic.gc.ca/enquiries-renseignements/canada-case-cas-eng.aspx)

You certainly can land without status. They are processing the case under public policy. That means you cannot be refused a PR just because you are out of status. So the letter is a little misleading. Being in AB, that's where they process OWP's. So, you might get it a little sooner -- but not likely before you'd be ready to land (meaning you'd have to land out of status anyway).

Net effect: the law literally states you cannot be refused a PR just because you are out of status (there has to be more to the decision). So, don't worry!

Here is the processing manual: http://www.cic.gc.ca/english/resources/manuals/ip/ip08-eng.pdf
Pg 64 - 75. It outlines how they have processed your case (as you ran out of status).

You laugh -- but it's $600 different. $760 vs $1366....
