Details of visit to Canada
1. Purpose of visit: Is it returning worker?
2. Indicate how long you plan to stay:
from date: is it the date of application or the date I first arrived in Canada?
to date: what do I input here? should I put one year from the date of application?
3.Funds available for my stay (CAD):Is there any specific minimum amount I should put?
4.Name , address and relationship of any person or institution I will visit:
Is it mandatory to provide 2 names or one is enough? Is it ok if I give a friend's address or should I give my office address?
Employment details:
Is it ok to give current employer's details? Since I am applying for visa based on open work permit, my concern is that they should not issue a visa again on the current employer.
UCI: I read in some other posts that UCI will change on BOWP. I am still waiting to receive my BOWP in the mail. If it is a new UCI, then I need to put that UCI on this form. Am I correct?

As per my experience Online is easy and faster way.
First complete the questionnaire and then it would ask you to fill one or forms, then Payment.
You also would need scanned copy of your BOWP, all pages of Passport and a Passport size photo. (as you just applied for BOWP so all these things should be already available to you)
Once the application is approved (it would take around 1-2 week), CIC will generate a letter (request for passport) and upload to your application.
You need to print this letter and send your passport along with this letter.
Make sure to keep a pre-paid envelope by writing your written address in your package so that they can send your passport back.
You would receive passport back in around 4-5 days.
