I was in the same situation. I couldn't do medicals for my son who I had with my ex-partner. I proceeded with the medicals with my family and the medical officer sent our medical results to the visa office the next day. About a week later, I got the letter above from my visa office acknowledging the receipts of my medical results.
I would advise you go ahead with the medicals for your family members and after that send them a CSE explaining the reasons why you couldn't do the medical for your step child. Most likely, you will receive instruction from your visa office to (1) send the evidence of your separation and (2) you will be sent a form(R117) to sign that you will not be able to sponsor the child in the future.
