Hi,

I'm filling up the family citizenship application forms, and have a few questions:

1. When specifying address and work dates in the form - do I leave empty the end date of the current work/address, or just fill the same date the application is signed?

2. Do I specify children education history in the application of both parents? I hope I don't need two copies of school records..

3. In children education history, do I specify current year school? What is the end date then? I can't provide a school record for the current year..

4. I'm supposed to attach the biographical pages of my passport, but also two pieces of identification. Can I just attach a driver's license as the first identification document, and use the passport pages (and the PR card photocopies) as the secondary forms of identification?

Thanks all!

Answering myself, after talking to CIC:

1. type N/A for the end date of current position/school year
2. one parent is enough, doesn't matter who.
3. yes, specify current year, end date NA, have to provide some record like an enrolment approval.
4. I need to different types of ID, like health card or tax record as long as it has my name and birth date and is current (last two years).
