Any one knows if I need to fill up the travel history in my application if i check yes in the E. CONSENT TO DISCLOSE INFORMATION

23. a) History of entries into Canada

(By selecting "YES" in the box below, and by signing this form, I hereby give my consent to the Canada Border Services Agency (CBSA) to disclose the details of my history of
entries into Canada to Citizenship and Immigration Canada (CIC) in order to assist CIC in determining my eligibility for a permanent resident card.
I also hereby authorize CIC to collect the history of my entries into Canada from the CBSA.)

yes you need to accept it. that doesnt clear any doubts about your stay in canada.
 
Thank you for the answer but the question was, if accepting this section I don't need to fill up the part where they ask every single entry to the country or I still need to do it?
 
you need to fill out as well and accept.they will verify whether you are right or wrong?.only include entries 5 years back date from now

You aren't required to consent, or answer. If you don't, they aren't required to give you a PR card.

If you want a card, consent and provide history.
