If you go for upfront you do that in your own risk. If you have family members included in your application then I would suggest you wait, if not, then go for it.
What are the advantages & disadvantages of doing upfront medical?

What is the risk you mentioned above?

The only advantage is time. When you get Medical Request from the embassy it will take some time to make an apointment at the designated clinic. The risk associated with doing upfront is that you do not know whether you'll get to that stage. There are cases when applications are rejected during background & criminal checks resulting positive, misinterpretation etc.

Also in addition to what Sookie85 has said, Every medical is valid for a period of one year, and u might be asked to go for another medical if ur application isnt finalised within a year from time of upfront medical.

It can also put pressure on ur Landing time if u are an outland applicant as ur medical musnt expire before u land in Canada.

Thank you for your advise.
Actually, I am frustrated due to this killing wait and above all I know most of people with AOR in December 2015 already done medicals.
How we can push visa office. remember I am out land applicant

This is one point in my mind and that's why I am hesitant to go

No reason to be frustrated. You are within the timeline. I got MR only after 11 months.

What is your opinion?

I visited my local VO last week and was told the following about upfront medicals:

"It is a major problem when people do upfront medicals as they have to go into a system where ALL OF THE WORLDS upfront medicals are and we then need to link each medical of each person in the application to that application which takes a lot of time and can result in your application being delayed."

I would strongly recommend that you do NOT do upfront as this may complicate/delay your case and there is NO PROOF that it actually speeds up the process.

I went for the medical and it was a breeze!

They did a very basic medical examination:
1) Checked our weights
2) Checked our lengths
3) Checked our throats
4) Listened to chest/breathing
5) Urine sample
6) Blood sample
7 Chest ex-ray

Was done and dusted in 2 hours time :p

Hello everyone!

I have received AINP and planning to file application with Federal for Permanent Residency. I am in a confusion
regarding signatures on PR application and other forms. My current signatures are slightly different from the passport signatures which is 8 years old. is it a issue ? If so, please suggest me a solution for the same on urgent basis.

regards,
