Go back to Doctor's clinic where you made appointment and ask for the document. Clinic receptionist will give you a document which will have First name, last name, passport number and UMI. That's what we should scan and send to CIC. If blood lab people takes it from you, you should go back to clinic and ask for another copy.euphoria nostalgia said:Hi guys,

i just got back from mine and the clinic gave me an emedical document attached to my blood work form. i thought it would be for me to keep but the guys at the blood lab said they have to keep that document.
so i have no document to scan. i'm not sure what to do now. any ideas will be appreciated. Thanks guys
