There must be some sort of error as the form was there before. I noticed the page was updated as they mentioned it was Last Updated : 2015-06-18

Send them an email at immigratepei@gov.pe.ca to ask about the form.

Hello has anyone received an update on their application since expressing interest through the PEI PNP Express Entry Stream?

I got an email 3 days ago from PEI saying that there are few jobs open for software engineers. They had attached a list of 30 open jobs. We just had to send our resume to them via email and they would then forward it to any open jobs in PEI.

Apart from this, no updates yet.

You agree to settle in the Province that nominated you. See CIC Schedule 4 document. If you do not wish to move to PEI, apply elsewhere.
Actually, during the application processing, CIC can deny an application if they believe you don't plan to settle in that P/T.

I would say apply to the Province you wish to live in. They are nominating you for a reason.
