That is not correct. Option C is for each Tax year which in Canada is Jan-Dec therefore it is calendar year not the last 12 months. Option C printout will reflect the information on NOA.

I agree with you, all the ones that I have requested for myself for sponsorship or super visa have always been based on my Notice of Assessment which is obviously for a calendar year which is also the tax year.

Hello all,
My parents are here with me in Canada under super visa. We submitted the medicals as part of PR sponsorship last month. Hopefully, the next step would be passport request. The PR application is processed in New Delhi. I am wondering how to submit the passport when requested by CHC in New delhi. It is illegal to mail/courier passports out of the country. I do not want my parents to travel all the way to India just to get the PR sticker. Is there any other way to get it done? Any suggestions will be greatly appreciated. Thanks.

Hi. My parents are currently here in Canada while their PR application is being processed in LVO-Manila. They now received passport request and was given an option to send their passports to Ottawa. I would guess your parents will have the same option as well. Please see excerpt of instruction for your reference:

3. Submitting your passport(s) in Canada (CPC-O) – if applicable
If you and all the accompanying dependents on your application are currently in Canada and plan to be in Canada for at least three months, you may submit your passport(s) to the Case Processing Centre – Ottawa (“CPC-O”). You are required to send your passport(s) together along with a copy of this letter by mail or courier to the following address.

If sent by regular mail or Canada Post Xpresspost (recommended option)

CPC – Ottawa
PO Box 9780
Ottawa, ON, K1G 4B9
• Purchase two prepaid letter-sized (32 x 24 cm) Canada Post Xpresspost envelopes (Regional if you live in Ontario or Quebec; National for other provinces and territories), at any Canada Post retail outlet.
• Complete the mailing label showing your full mailing address in the “Deliver To” field.
• Make a note of tracking numbers for each envelope.
• Enclose both envelopes along with your passport and a copy of this letter.
• You can purchase a Canada Post shipping label on line: http://www.canadapost.ca/personal/tools/cst/intro-e.asp
If sent by private courier (only)

CPC – Ottawa
365 Laurier Avenue West
Ottawa, ON, K1A 1L1

• If you choose to not provide us with a return Xpresspost envelope, we will return your passport(s) via regular mail (Canada Post Lettermail). Passport(s) will not be returned by private courier. Do not provide any pre-paid courier envelopes: These will not be used.

i think i may have mixed up with something, cic usually ask last 12 month income when they asked for reassessment and also ask for the option C print out so i may have thought that the option C is last 12 month. thanks for the correction.

While filling personal history in schedule A form.
Where they asked for no gaps.
Can I attached a word document for point 9 because of lack of space

one more question.
I have 6 months gaps in studies. Should I write unemployed or at home. ?

I'm planning to have upfront medical for my parents, my father is in Canada while mother is back home. Can they do medicals from different physicians and what is the procedure for that. Anybody who have done similar please guide?

Also what is the fee for medical examination in Canada?

You'd have to call the doctor's on CICs panel to find out the cost.

Yes family members can do medicals with different physicians if they're @ different locations.
Make sure they take file #, and their UCI # when doing medical so that it can be matched to their files.

thanks canvis2006, can somebody confirm mailing address of London visa office who have recently submitted any documents to them?

Thanks.

Every doctor charges different fees. I called few and Physician in Hamilton was charging $ 255/person. However, physician in Kitchener was charging $ 195/person. Check with the doctors office.

Also, can you tell me which forms London office requested of you beside the medical? We are doing upfront medical and I am thinking of sending the receipt of medical along with updated forms to London office by Wednesday.

Thanks
