If you guys need an option C print out you may print off from CRA website by registering first, call at 1-800-714-7257 keep your tax papers handy as agent ask some security questions.
 
Hello,
I have a question. My parents just got their medicals done in India. The DMP office gave them some papers and asked them to send it to Ottawa telling that these papers will let Ottawa office know that they got the medicals done.

Did any one else's parents got some papers like that?

Thanks Starzibal. Can somebody also let me know if we have a template for affidavits for non availaibility of birth certicate and marriage certificate for parents..

or should they just have something like

Birth Certificate

I ------ swear that I was born on XXXX date YYY City ZZZ country and no birth certificates were availaible at the time.

Marriage Certificate

We ------ and ------ swear that we were married on XXXX date YYY City ZZZ country and no marriage certificates were availaible at the time.

Any help from somebody who has submmitted these would be really helpful...

You can go to any oath commissionair/notary public office, they know about these documents and will prepare for you. You don't need any template. They would have everything, just tell them your parents info e.g. dates/names etc.

we are registered with them, is it possible to print option C print out on-line from their site?

If you are registered already then yes you can print off option C from their website.
Just log in and see the option on the website. I am not registered yet but will. I just called so CRA agent told me that.

I think first you need to create your account here: http://www.cra-arc.gc.ca/myaccount/

I just register and they will post a security code to me in few days.
