you (your parents) will be given a form with your photo, you give the form to a doctor, the doctor is supposed to ask all the questions and fill in the form, then you sign the form confirming that it's true... you are not supposed to fill the form yourself... but you are signing it...starzibal said:I don't recalling filling up any forms. Passport is required (even within Canada when you do medicals for immigration). However, I know that in Canada, for example, you can substitute the passport with another identity document with photo. Ask the doctor who will be performing medicals. that's wired for permanent residency it should be date FEB 13 2012
