is there another name for this form? Because i requested my option c printout from revenue canada, and the form I received doesn't say "OPTION C" anywhere!! It just says " INCOME TAX INFORMATION- REGULAR"

the last year I paid canadian taxes was 2002 (2001 tax year) since I've been living outside of the country.

I'm just going to include this anyways, but would like to know whether I should call it something different in my paperwork explaining why I don't have an option-c for 2009.

anyone have any advice?? am I freaking out over nothing??
 
mine didn't say the words Option C Printout either - actaully, I got it mixed in with other tax documents in April and almost had to request another one because I wasn't sure what I was looking for!

If you requested the Option C and that's what they sent, then include it.
 
The option C printout we got says "Canada Revenue Agency" across the top and RC143 F (04) above each of the page numbers. It is a three page document that starts with a box containing his name and address, SIN number, birth date, province, Tax year, marital status. Underneath it has printed lines of text beginning with 3 or 4 digit numbers then a description, amounts etc. It's in French so I don't know exactly what's included. At the bottom of each page it has someone's initials, the date and some other numbers.

ok phew! I dont know why a spasm of uncertainty overtook me....this process sure plays games with your head!

yes, the form I got says RC143 E(04).

I will just note as being my most recent Option-C printout.
