The first VO said review required on the assessment at the end of the notes. at the same page in the top right corner, they also have the s.16(1).(c) , and S.19(1) written there.
I want to believe the second VO should have review by now.

eCas shows IN PROCESS since October 9, 2015 however the file officially went IN PROCESS March 02, 2015.
and initial review note says Eligibility: REVIEW NEEDED since July 23, 2015. Here is the reason why it says review needed, an officer has not yet finished working on your eligibility yet. I see you have s.16(1).(c) , and S.19(1) on the right hand top corner of your initial screening page. What does S.16(1). (c) says:
S.16 (1) The following heads of government institutions shall refuse to disclose any record requested under this Act that contains information that was obtained or created by them or on their behalf in the course of an investigation, examination or audit conducted by them or under their authority and (c): the Information Commissioner; and
To better grasp what is (c): go on google and type the "Information Commissioner". Also at the time you ordered your GCMS notes it is clear that BGC was not initiated yet hopefully it has started. there are some investigation going one as you can see. You will have more clarity when you get your second GCMS. If you still see the same thing it means they are still investigating something about you, when they will wrap up they will remove those codes of information acts.
Anyways did you sponsor someone in the past to Canada? I remember before I sponsor my current spouse I did a previous sponsorship which I canceled long time and every time I used request my wife's GCMS there used to be S.19(1) XREF then a blank space like I did. I found out in the notes that the Visa Officer who did my initial screening was talking about my previous sponsorship of my ex then (XREF) will follow then a blank space. I understood that it was the file number of my ex which CIC did not want me to see because it is someone's personal information which I was not supposed to see. Hope you get the reason why I asked if you had previously sponsored someone to Canada. Or you might had a girlfriend in Canada before your current wife and you had a child with? well I was just guessing or bite your memory if you know why they are hiding personal information from you.
When the officer will complete your eligibility he or she will mark it as "Recommend Pass" and Then they will put it in the final review queue. In order to better understand your file, check your initial screening notes once more. Let me ask a question, does the officer have any doubt about your relationship? You know what the officer has already said in your GCMS about your file. Here comes a catch, It might take a bit long to complete the final review on Eligibility in case the visa office is experiencing backlog and lot of files are in queue for final review as well. There are decision making officers who will review these files for the last time and they will either mark on them "eligibility PASS" or "REVIEW REQUIRED" .
CIC will request medical extension to their discretion and if they don't, just wait they will contact you about your medical status.
