- when I open IMM1344 field co-signer is marked red and asked to fill out.
- IMM 0008 I filled out online but i didn't filled out for my daughter from my previous marriage
- how can I add IMM0008, IMM 5669, IMM 5406 for her, she is 21 living in Germany and has no intentions on moving to Canada

• IMM 0008 Generic Application Form for Canada: A current, completed and signed Generic
Application Form for Canada (IMM 0008) has not been provided. The principal applicant must
complete this form for themselves and dependents. If you require additional space to list
dependents, please use the IMM0008DEP (version 06-2020 or later) form.
• You must submit the most recent version of this form and it must be properly signed.

• IMM 5669 Schedule A (for Dependant): Please provide a current, completed and signed
Schedule A (IMM 5669). This form should also be completed by all dependents aged 18 and
over, whether accompanying or not.

• IMM 5406 Additional Family Information (for Dependant): The Additional Family
Information (IMM 5406) is missing or incomplete. Please provide a current, completed and
signed Additional Family Information (IMM 5406) for all dependents aged 18 and over,
whether accompanying or not. Each individual must complete and sign their own form.
• Sections A, B and C must be fully completed for this form to be considered complete. If you
are unable to provide any of the information for any individual listed in section A, B and C, you
must provide an explanation. You must submit the most recent version of this form.

• Police Certificate: A Federal police certificate is required from the country in which you have
spent the majority of your adult life (since the age of 18). Please visit

Centre, toll free, at 1-888-242-2100 for further information on how to obtain a Federal police
certificate. Note that regional, county and national police certificate are not acceptable. Kindly
ensure you provide a Federal Police Certificate as indicated on the IRCC Website. If the
police certificate is not in English or French, a notarized (certified) translation in either English
or French with a copy of the original version must be provided.
• Birth Certificate: Please submit a Birth Certificate or Registration document for your child
which indicates the names of both parents.
• IMPORTANT: If your documents are not in English or French, send a notarized (certified)
translation with a copy of the original version. To have a photocopy of a document certified, an
authorized person must compare the original document to the photocopy. Neither you nor
your family members may translate or certify copies of your documents.
