Thanks for the notification especially on daramz who seems not active on this thread.I'm following up on him.I have actually contacted him inviting/telling him to join this thread.I'm hoping he will join us soonest but the most important thing is that we are sure he has been contacted with reference to his medical request unlike "Eagle1" wo we are yet to hear from.

· Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature updated for all family members over 18 years.
· Police Certificate: From each country/state where the residence period has been six months or longer since the age of 18, provide an original police clearance certificate.
· Complete or provide proof of completion of the Immigration medical examination for yourself, and all family members. The information for this will follow by email shortly.
· Additional Family Information Form (IMM5406) fully complete by your spouse

If i am right...you are referring to the academic request form for WES or one of the evaluation bodies for your ECA(correct me if I'm wrong).
If yes,you will surely get your reference number once you register and pay your fee for the evaluation.A reference number will be automatically generated for you which you can write on your academic request from before giving to your academic institution to include with your transcripts to be sent directly to WES on your behalf.
Its always better you fill it before giving to the academic institution officials......Even if you dont fill it and you gave it to the officials and they sent it with transcripts,WES can pull your details out of their database if you have truly paid your evaluation fee.

I was also contacted by AVO yesterday and they asked me to provide the below within 60 days:

* An Application for Permanent Residence in Canada [IMM 0008] for you.

* Updated the Schedule A: Background/Declaration [IMM 0008 SCHEDULE A] for you and your spouse respectively

* Updated Additional Family Information form [IMM 5406] for you and your spouse respectively.

* Payment of the Application for Permanent Residence Fee for your new dependent if he is accompanying.

* 4 passport-sized photos for your new dependant, as per the photo specifications found in Appendix C of the visa office specific instructions.

It looks like there is an issue. No form called IMM 0008 SCHEDULE A that is referred to as Schedule A: Background/Declaration. Please those that have had additional members to their family and have gone through this process, please share.

Just to add that there is no IMM008 Schedule A form. I guess it is just a typo. Please fill the IMM5669 Schedule A/ background declaration form. AVO sent the same thing to me and I almost went mad looking for the form.

@ kit01,from the best of my knowledge,Kay_diwe is an FSW_2014/2015 applicants who's application was received in CIC on 26th May,2014 and got his PERS 17th Sept,2014.He belongs to the current stream of FSW and 2013/2014 stream.
There was an addition to the family which made the applicant to contact Accra to be able to add the new born to the application package.
Seems Accra just responded to the request which was shared for us....but because we are all caught up in MR happy mood,its confused for medical request......Its not and i'm sure the applicant will clarify this out.

As you have been told,the IMM 0008 SCHEDULE A is just a typo error.
They actually meant IMM5669 Schedule A/ background declaration form.
Complete this form each for you and spouse and include it as part of the documents requested by Accra Visa office and send by courier to the address given to you.

Please praise Almighty God with me as we received our MR few minutes ago. It came in around 3:30 pm (Nigerian time). This can only be God.

We were only requested for the following:

1. RPRF ( for me and my spouse)

2. Schedule A form (IMM 5669) which is background declaration form for me and my spouse

3. PCC: for me and spouse

4. Medicals: for all family members

5. Submission of documents including medicals: within 60 days


NO POF !
