Hi friends

In the guide provided by CIC specific to India, The "SUPPLEMENTARY INFORMATION FORM" (on page 6 of IMM3904E) asks for file number #. What is it?

Another question my friends.

I am a Canadian citizen now. My wife is from India. We got married in December 2015. My wife and I were never married to anyone before, and we do not have any children. My wife is populating the checklist for the 'sponsored'. Below are the forms she is filling up from her side. ('Yes' meaning she is filling, and 'No' meaning that she is not filling).

IMMIGRATION FORMS
Generic Application Form for Canada (IMM 0008) - YES
Additional Dependants/Declaration (IMM 0008DEP) - NO
Schedule A - Background/Declaration (IMM 5669) - YES
Additional Family Information (IMM 5406) - YES
Sponsored Spouse/Partner Questionnaire (IMM 5490) - YES
Use of a Representative (IMM 5476) - NO
Supplementary Information Form (Details of Foreign Travel) - YES

As for the checklist for the India specific, she is using the IMM3904E document for the India, and putting tick mark for the "yes" ones, and N/A for the "no" ones for the item 1.

The thing is that this document 3904 does not have a checkbox next to the forms "Schedule A - Background/Declaration (IMM 5669)" and "Use of a Representative (IMM 5476)" in the item 1 "IMMIGRATION FORMS". Not sure how did CIC miss that.

Can you please share your inputs on how you dealt with it?

If you are sponsoring your wife - you don't need to fill this form. Only the principal applicant needs to fill this form.
The confusion arises because this form gets used by other types of immigration candidates (apart from spousal sponsorship).

Thats right, include Document checklist IMM5491 in Sponsor's Envelope and Information Guide IMM3904 in Sponsered Spouse's Envelope

2. The thing is that this document 3904 does not have a checkbox next to the forms "Schedule A - Background/Declaration (IMM 5669)" and "Use of a Representative (IMM 5476)" in the item 1 "IMMIGRATION FORMS". Not sure how did CIC miss that. you are right and if u see closely CIC missed out on Schedule A Background declaration IMM 5669, there is no check box for this too. But I believe as long as u put check marks in document checklist IMM 5491 and include all the above mentioned forms in respective envelopes all should be fine.

BTW have you got your "Notice of Assessment" for most recent year. If yes, how long did that take. I am in Toronto and planning to apply for "notice of assessment and Option C" next week.

Does our e CAs status change when we receieve our AOR 2?? As sometimes we do not receieve e mail so. Can we check AOR 2 online ?? If yes then please guide me as to what will be written ????

Hello Drkdd,

according to New process adopted by NDVO since June 2015, your eCase will change to in process from application received...

I left it blank as my wife (principal applicant) didn't have a file number at the time of applying.
I think you have to leave it blank.
