To answer regarding the missing checkbox... We had the same thought as you did.. After pondering on it for five minutes, we drew our own checkbox (where there should have been one) and put a tick if it was yes, and N/A if it was no ..happyenium said:Another question my friends.
I am a Canadian citizen now. My wife is from India. We got married in December 2015. My wife and I were never married to anyone before, and we do not have any children. My wife is populating the checklist for the 'sponsored'. Below are the forms she is filling up from her side. ('Yes' meaning she is filling, and 'No' meaning that she is not filling).
IMMIGRATION FORMS
Generic Application Form for Canada (IMM 0008) - YES
Additional Dependants/Declaration (IMM 0008DEP) - NO
Schedule A - Background/Declaration (IMM 5669) - YES
Additional Family Information (IMM 5406) - YES
Sponsored Spouse/Partner Questionnaire (IMM 5490) - YES
Use of a Representative (IMM 5476) - NO
Supplementary Information Form (Details of Foreign Travel) - YES
As for the checklist for the India specific, she is using the IMM3904E document for the India, and putting tick mark for the "yes" ones, and N/A for the "no" ones for the item 1.
The thing is that this document 3904 does not have a checkbox next to the forms "Schedule A - Background/Declaration (IMM 5669)" and "Use of a Representative (IMM 5476)" in the item 1 "IMMIGRATION FORMS". Not sure how did CIC miss that.
Can you please share your inputs on how you dealt with it?

I am waiting for SA at this moment and didn't get any reply with missing or incorrect filling of forms, which means - they were okay with us doing that.
