The thing is that this document 3904 does not have a checkbox next to the forms "Schedule A - Background/Declaration (IMM 5669)" and "Use of a Representative (IMM 5476)" in the item 1 "IMMIGRATION FORMS". Not sure how did CIC miss that.

Can you please share your inputs on how you dealt with it?

To answer regarding the missing checkbox... We had the same thought as you did.. After pondering on it for five minutes, we drew our own checkbox (where there should have been one) and put a tick if it was yes, and N/A if it was no :) ..
I am waiting for SA at this moment and didn't get any reply with missing or incorrect filling of forms, which means - they were okay with us doing that.

Have you already submitted the application ? - If not, just wanted to say - they don't require Option C form anymore. Option C form used to take about 5-7 days to come in mail, so if you haven't submitted the application and are waiting on that; then don't wait on it.

Notice of assessment - you can download it from your CRA account online.

Just thought of mentioning it.

Thank you canadavisa1986 for your response.

Option-C is not needed anymore as per the new checklist revised in Feb 2016. We just need notice of assessment that I believe can be downloaded from the CRA site. However, I plan to wait for the official copy of "Notice of Assessment" from CRA for a few days, as I was told by my tax consultant that it would take about 2-3 weeks to get it in mail/post from CRA.

Thank you Aamm for your response. That's a good idea. I plan to have my wife draw a thin line corresponding to the form name when the checkbox is not available when it should have been made available, and put a tick mark or N/A accordingly.

I think you have to leave it blank.

I would think writing 'N/A' is a better option, instead of leaving it blank.

I have a questions about the form IMM5475 (Authority to release personal information to a designated individual). If I have to contact CIC about information of my wife's application, does she have to fill in this form? Have you guys filled it?

Hello,

I don't think you, as a sponsor, have to get it filled by your wife. As you are the sponsor, you will have access to information to view the progress.
The form is for a third party.

That's what I think. Maybe someone else can correct if I am mistaken.
