As long as you tell CHC Islamabad (which I believe is your embassy) about this before they find out you will be fine.
Waiting2Exhale is our first choice for stuff like this as he might already have helped someone with a similar issue, and most likely he still has the format for the letter and other information that you might need.
Send him a message in his inbox, if you don't hear back in 10-12 hours anyone of us will be glad to help you out.

Dear Visa officer,
Reference my UCI # xxxx, original application date : (Date on which you first sent the application), for category: Family Class Spousal Permanent Residence sponsorship, I would like to firstly apologise for an error committed by me in the Form (Title of the form, Form number) wherein I inadvertently submitted ‘No’, to the question asking ‘Whether I have applied for visitor visa to any other country in the past’.
In fact, back in 2007, I had applied for a visitor visa to the U.S.A. It was an genuine error and oversight from my end for which I apologise once again and would like to re-submit the corrected and duly filled form once again, found attached along with this cover letter.
I trust you will consider this form instead of the previous one and also let me know in case you need me to do anything more on this matter.
I sincerely appreciate your understand in this regard.

Fastest was in 138 Days (Himzz), and slowest in 586 Days (DeepSingh).
This shows that NDVO can reach their 6 months target 180 days. They should have kept 150days average as the yardstick to keep the VO on their hooks. Trust when they update process time next let it get reduced to 10 months.
Let us wish every one to get Visa in 150 days at least 2014 applicants.

NDVO can set their new best with 2014 applicants, and I hope they do so 2014 guys don't have to suffer as much.

Dont worry its not something really bad. Its just a mistake and as long as you figure it out before they actully point out its always good. Fill the form and write a short letter stating you made a mistake and here is the correct form you r sending. Things will be back on track for you.
 
Hi everyone........we received remedical request from cic with an attached remedical filled form.......and attached letter......and ask to send it in 60 days....
its written in the letter clearly that once you have completed ur medical.....the doctor will forward it to cic...........so I am worried now that do I have to send them the copy of medical form or not.......though my lawyer told me that its not necessary to send anything.....but u want then can just email them......
m so confused....do I have to email them......though doctor said that u don't have to send anything.

don't have to send anything...but if you are paranoid like me, just send a copy of fee payment to NDVO with a note mentioning when examination was done; your file number; applicant's name to their physical address....even this one is not needed but just to sleep well at nights and not worry unnecessarily, you may want to send a copy of fee receipt...
