Exctly same note before 3 days.
Only 3rd point is minor different.
1st and 2nd same to same...
In mine 3rd point is "CSCD not entered as missing the original Imm1017; only the emedical notification was submitted".
Today i meet my local medical hospital dr, and they said its not any problem.
So dont worry dear...

Or CIC finally pissed off an attacker and he did dos attack on server. Lol they are using apache 2.2.15 looks like and is easy to attack
