Need help to fill Address history details for e-APR

Dear Experts

I need some help in filling the address details in my e-apr form.

Provide all of the addresses where xxxxx has lived in the past ten years or since xxxx 18th birthday, whichever is most recent. Do not leave any gaps.
Check the list at the bottom of the form first to see information you have already saved.
• When you click "Save and Add", each address will be saved to the list.
• The fields will go blank to let you add details about another address.
• Use the modify or delete button if you want to change details that you have already saved to the list.

So my situation is

I was living in my home country since my birth till march 2012 when i moved to Canada.
Then i came back to my home country in Dec 2012 on my same home address,
Then I moved to Canada again in May 2016 and returned back to my home country in June 2017.

So what should i mention in my application about my address history?
Is it something like this:

1 . 1st june 2017 -- current address -- YYYYY
2. May 2016 to May 2017 -- Canada address
3 Dec 2012 to May 2016 -- Again my same address in home country -- Here should i mark the field "current address as yes or no"
4 March 2012 to Dec 2012 -- Canada Address

Please help.

the current address means your present home address
