There's an Appendix A from CIC website that you can download. We used that and it worked. We told CIC that we never got the request. Also called the other day to have it resent but still nothing. We do not want to delay any longer so we have to do something to speed things up. We told CIC about this but they didn't say anything, so I guess they don't mind. Well it's on their website available for download to the public anyway. We asked for extension and it was granted "internally". It is most likely a request for your Schedule A and police certificate. You will either have to upload them through the IRCC Webform, or mail them to this address:

The police certificate is fine as is, but the schedule A form you will have to Validate, Print, Sign it by hand, Scan it, and then submit it through the webform or mail it, whichever you choose.
Hi can you kindly advise after how many days of uploading schedule A via IRCC webform did you get the confirmation email from them that they have received and attached it to your file. I received that reply after 12 business days. We have something like 45 days to submit the documents. From what I read here, I thought the time given to submit the documents was much shorter then that

We called the Immigration office yesterday to check on our application. They said they haven't even opened our case yet and thats why we dont have our file number

I filed on Mar 2nd and no medical request or news after sponsor approval whatsoever. Because I am used to the processing scheme of CIC, I regard this as a good sign. A refusal is the quickest of all responses.
