5. CITIZENSHIP
6. FULL ADDRESS/ES FOR LAST TEN YEAR PERIOD (including present address)
7. OCCUPATION/S FOR LAST TEN YEARS (up to and including present occupation, full details)



I never seen the above format previously. Is it very common thing ? Is it like additional Information chart or PPR1? I am bit worried. I submitted updated schedule A form then what is the meaning to ask the same information again?

Awaiting for reply.

I think Schedule A focuses more on the principal applicant while the information LVO want from you is regarding your family members over the age of 18. I cannot see any other reason.

Congratulations JohnnyZee. We must have passed each other without knowing. I was at the vfs London this afternoon to submit my passport too.

are the contents of the additional information chart same as supplementary information form which I received?

Contents for information chart are just:
Surname, (as in passport) First Name, (as in passport) Passport Number Passport Expiry (dd/mm/yyyy) Height (cm) Eye Colour Code (see chart below) Native Language

Yeah, we prob missed each other. I am hoping it will be done within 2 weeks but who knows what LVO is up to!

2 weeks is too much to get the passport back to us. I reckon in 5 working days, by thursday next week, we should have our passports back (friday at most). My friend got his passport back in 5 working days.

Hi moncherie, initially it was DXS but received request for med, pcc, rprf and PP from RIH.
