Yes for months ago LVO asked for MR 4 - 5 months after Application Receive date BUT you should check when have they started processing them? I am sure that start processing date must be close to medical request date. because when they active one case, they should finish it soon! ;)
But also there are some applications need extra background and security checks and for those "In Process" may take longer.

yes you are right its logical if they started process on file its not affordable to take it too long . but on the other side we should check the process start time after app rec to start in process and then MR... thats a good point

i saw some timelines of LVO that after AOR about 4 to 5 months to take staus change from receive to Inprocess and then after the date of in process it took 14 to 20 days to get MR point is back in 2014 it took 4 to 5 moths to second line update but in my case i got second line update after 15 days of AoR


any opinion ??

It means, you should prepare yourself for MR because it is close to you!(Up to 1 month after In Process)
With Starting your file's process, They are really working on your file and you can receive your visa in next 3 - 4 months! ;)

ya hope for good but im an outland and cant expected or realize that movement by LVO

Do you know what exactly his/her NOC is? Because for PNP program the minimum IELTS requirement is CLB4 and they only can ask such a strange request that applicant's NOC and intended occupation NEEDS high English language skill.

As cic raised minimum levels in the recent years for O, A and B categories, they are asking on it i guess. Possibly its an audit kind of thing which is not for all and regular one. So nothing to worry. Every case differs hence things will be fine for others. CIC should instruct pnp to have minimum level instead of asking candidates at this stage. we cant help as cic and visa officers have equal power when it comes to application adjudication.

What's the applicants age and number of dependent ? Sometime if the applicant is overaged or has overaged dependent than the officer may doubt on his/her ability to establishment in the province. Can you collect those information as age and dependent condition is a vital issue here.
 
For those who like to find out how many months will their PNP applications take to be finalized these days, I have a Scenario:

I think this scenario can represent their workflow for current 45 days processing time frame at CIO level for Outland applications:
1- They do their completion check within first 30 days and if any signature is missed or something is wrong with application, they will return it to applicant from 30 to 35 days.
2- If one application is OK and passed previous level then they will do their validity checks for provided documents for example for nomination letter, payment receipt,... from 35 to 40 days
3- They finalize applications passed two previous levels and they insert it in their online system and get UCI & file numbers from 40 to 45 days.
4- They send AOR to applicant from 45 to 48 days!

And I think After CIO level these could happen next at Local Visa Office level:

1- CIO transfer applications to their assigned local visa offices within next 3 weeks after EP AOR date.(Transfer)
2- Visa Office will start processing received applications within next 1 to 2 months after application delivered to visa office (In Process)
3- They send Medical request and RPRF within next month after they started processing application (Medical and RPRF request)
4- They receive Medical Results within next month after medical request date If applicant go to do medical without any delay. (Medical Result Received by Visa Office)
5- They will finalize application and send passport request within next month after medical result and RPRF received (Passport Request)
6- They issue visa and stamp passport and send back passport in next two weeks after they receive passport! (Visa Issue)

The whole processing time will be 45 days at CIO level and 3 months(MINIMUM) to 6 months (MAXIMUM) at Local Visa Office level (because any of rows 2,3 & 5 can be done in few days!)
