It says to check off only one of the four boxes. When I select 4C, it clears out 4A and vice versa.

I apologize... I did mine on old forms which allowed you to choose more than one. Nev sounds like she has the right answer

Yes, the form changed as of January 1, 2013 and you more than likely filled out the previous one. I asked a few other people on the forum the same question Halfmoon did and they suggested 4C. I still haven't received my notes so who knows what I'll get!!!!!

Thanks. Yes I was aware they changed but I unless you fill it out online I guess you won't know you can't choose more than one. If u print out the form can manually select two options? (Maybe not as halfmoon said it states to "choose one") I should have double checked the new forms before answering.

lol!!! Sorry, form IMM5563 didn't change; however, form IMM5744 (new rule effective January 1, 2013) needs to be signed and submitted to give consent to access personal information because they are no longer accepting IMM5475 or IMM5476 in this instance. The instructions are all on page 1 of form IMM5563.

well the boat wont be exclusively for us - not unless i had a guest list of 60, and $ upfront.

I HAVE put on cruises for past events......and for the same lil cruise, so I done know what am who im dealing with, etc....

TO CLARIFY that: its $30 per person - u can pay upon boarding.

The cruise includes, dinner, music a kick ass captain.......u cant bring ur own booze but u can buy what u want.....Cruise lasts 4 hours...and its quite beautiful in the day, so i can imagine how pretty it must also look at night.

Our hubbies would love it.

I have a pretty good business relationship with the captain------so id make sure to contact him prior to make sure he's ready for us LOL

Think it would also be nice for our men to get together...and just have mutual connections, of the trials and tribulation etc...basically we've pretty much all and all WILL be overcoming the same goal...so purpose is to celebrate everything we've worked so hard towards!!!! ---while getting a pretty little taste of Toronto.

I've always wondered... when they call (instead of sending letter or email) do they just tell you all the things to do and hope you remember? My worry is that hubby will get a call from POS and they will say you need to send A, B, C, D, and E and hubby will say ok and then he'll forget half of it and we wont send the right stuff... Like what if they call him and he's not somewhere with a pen.. Ahhhh! lol

I'd had the same discussion with my hubbie and I told him if he is unsure about anything they are asking to make sure he repeats to confirm not just just be timid and say OK.

Perhaps I'm going to suggest that he ask if they could also email him what they're are requesting so that he has everything in writing?

Great minds think alike! Hubby and I just had this convo the other day. Since then he has memorized his file number, carries pen/paper with him EVERYWHERE, and knows that we paid for everything up front (incase he is told we owe for something).

so the prints were done in st.mary and he picked up the completed checks in kingston and then dropped them off at embassy and also faxed them in

it took a week to complete them
