The Schedule A has to be signed manually but can be scanned and sent by email. Since you were asked to submit a new police certificate though (only originals accepted), you might as well send the form itself along with the new police certificate by mail.

I see you did two medicals, one in February, and the other one in May. May I ask the reason for doing more than one medical within the same year?

I got my medical request on Feb 17, 2015 and completed for my self. Since my wife was pregnant, her X-Ray was pending. My wife's x-ray was submitted to CIC on 19-May-2015 after her delivery.
I paid my RPRF upfront on 21-April-2015 before my wife's X-ray was done.
I got my 1st GCMS note on 15-June and confirmed that My Medical is passed; Eligibility: Recommend Passed and RPRF received by CIC.
After the first update on June-15, my file was not touched by CIC until I got decision made yesterday.
I never received any RPRF request.

I just received my COPR and noticed that 2 dates shown in it is inaccurate:
1. Original entry date; and
2. Last entry date.

Both dates above show my last year re-entry to the country when I was back from holidays. However, my original entry was in 2013 and latest one was this October.

I called CIC to do my landing appointment and the call center didn`t know whether it's a issue or not. He said to mail or contact directly CPC Ottawa.

Actually your "Original entry date" is wrong. I think it is some glitch in CIC COPR printing , They take both dates as last entry date, My friend had a similar Issue... At the time of landing( In land landing) , He asked the visa officer for correction & got new correct COPR after showing proof from Passport, Original entry date's stamp.

you can also email CIC stating your Issue.
 
Hi,

I received an email to pay RPRF fee on last Friday (11/04). It is clearly mentioned in the email that admissibility review (security, criminality and/or medical) was still ongoing. Any idea, when I can expect to receive passport request.

I need urgent help. I applied for CEC PR in June 2014. Last month I received 2 years extention on my closed LMIA based work permit so I have WP till Jul 2017. But I lost job today, am I eleigible to apply for BOWP ?

Typically there are two conditions you have to meet to be eligible for a BOWP:

1. You have received AOR for an ongoing PR application
2. Your CURRENT work permit expires WITHIN the next four months.

Your current work permit expires within the next 23 months, so technically you are not eligible for a BOWP. You can either try to apply for a BOWP and write a really, really good cover letter to explain your situation and why you apply for the BOWP. Or you find another employer who will apply for an LMIA for you.

I do have AOR and I also received query from CIC about intention move out of Quebec. y current WP is closed and allowed to work only for same employer and city Montreal. in order to look for Job now I need BOWP. Technically even though my WP is valid for amother 2 years but from today onwards itd nothing but a vistor visa. is this enough reason

Basically you should have applied for a BOWP before your last LMIA work permit would have run out. And this is exactly the reason why BOWP option is there, so you can integrate yourself better into the canadian labour market with an bridging open work permit.. this would have enabled you to move out of Quebec.
The BOWP is not meant for people who get terminated by their current employer but for prospective PR applicants to have more job opportunities.
You missed that opportunity and now you are kind of stuck. If I were you I would try to apply for the BOWP anyway...

I think you can file for BOWP and send a letter along with the application explaining why you need the BOWP. they will issue BOWP definitely.
But you need to maintain the legal status in Canada. That's it.
