IMM 5669 E: Schedule A - Statutory Questions

I answered "YES" to one of the statutory questions in my profile. Do i need to upload IMM5669E form along with a brief explanation in LOE?

The form apparently asks for all the details again that i have stated in my post ITA profile already. e.g. Addresses and Personal History. Do i need to fill all of it again? Plus, for Personal history, there are not enough rows in the form. Mine is a bit more detailed. what shall i do?

Hi Hamza, hope all is good on your end.

I know that this is an old question, but how did you go about with the form? Did you write everything from the online application onto the paper form?
