Hi Staralihaider,

I ordered Caips 3-4 months ago, but I received to letters, one is the FOSS Check and another one is just only shows the time history of my file. There is no BF'D in these documents. In FOSS Check, I could see my point but in Caips just only time history.

I think this FOSS Check is not common, what do you think?

I am a NDVO applicant. I am the principal applicant on the file and my husband is my dependent. We are soon sending a CAIPS Notes request through a relative in Canada. This thread has helped me attain information on how to apply for a request. As per that guidance, I have filled up the forms IMM5563B & IMM5475E. Below is the detail on how I have filled them up. Please confirm if this is correct and whether there is an error anywhere.

IMM5475E
Tick "I authorise CIC and Canada Border Services Agency to...."
1. Principal Applicant's Family Name
Given Name
Middle Name
2. Principal Applicant's DOB
3. Name of office where the application was submitted: NEW DELHI
Location of office: NEW DELHI, INDIA
Type of Application: PERMANENT RESIDENCE
4. Your Client Identification (ID): File number starting with "E......."
5. Name of my relative
6. Mailing address of my relative
Mobile number of my relative
Email address of my relative
7. Signature of applicant: My signature


IMM5563B
Requester's reference no. & ATIP File number: Blank
Surname, First Name and Middle Name of Principal Applicant
DOB of Principal Applicant
Tick the box "Receive copies of the records" under "Access to Information Act($5.00 fee)"
Tick "in English" under "Privacy Act (no fees), I wish to examine information:"
Tick "CAIPS (information abroad)" under "REQUEST FOR"
Write Canadian High Commission (New Delhi) by the side of the above line.
Write the Visa file number (E........) by the side of the above.
Tick "This request is being made by - a Canadian citizen, permanent resident or an individual present in Canada"
Write my relative's full name under "Name (requester)"
Write my relative's mobile number under "Telephone no."
Write my relative's mailing address under "Address"
My relative's signature

Some general questions:
1. Can I scan and email IMM5475E to my relative and he can take a print-out from there?
2. I am the principal applicant. Does my spouse need to sign in the "Signature of spouse or common-law partner (if applicable)" field in the form IMM5475E?
3. If my relative is willing to pay by Credit Card that he has to fill up this form IMM5667E (http://www.cic.gc.ca/english/pdf/kits/forms/IMM5667E.pdf), right?

I stay in India and so I do not want to err anywhere in the process - just to avoid going back and forth with the application and not be a trouble to my relative. That's why I wanted to confirm if what I have done is correct before sending the docs to my relative.

I think you will get medicals in next 3 months. :)

AND IN 2-3_2011 THEY SENT ME A LETTER ASKED ABOUT MILITRY QUESTIONNAIR IN MY COUNTRY.
WHAT DO YOU THINK? HAVE THEY FINISHED CHECKING MY EXPERINCE AND CERT? IS SECURITY CHECKING ONLY REMAINED?
