In general people really don't know how difficult it can be to get your spouse into Canada. I certainly didn't know it would be this hard before I tried. People tend to think that if you marry a Canadian you can automatically come to Canada and that there is no trouble or delay. Maybe it's because there are so many immigrants in Canada, and we hear so much about 'mail-order brides', that we don't realize that the immigration process can be difficult and time-consuming.
By the way, if you're applying as a spouse, common-law partner, or conjugal partner, you have to submit your medicals with the application. Too bad for you if your application process takes more than a year and you have to get them redone.
If a year has passed, I'd wait until they asked for the new medical. Sometimes the visa officer will extend the validity of the medical; and sometimes it takes more that two years, so doing it too soon is a waste of money. If you're worried that you won't be able to land within the two months or whatever that is left by the time you get your visa, I'd contact the visa office and ask if you can submit a new medical so as to extend the time you have for landing.
