Has doctor gave me the wrong medical exam results form?

I received ITA, qualified under FSW program, and I'm filling up the Express Entry post ITA form by uploading the files. My dependents and I went to one of the doctors listed in CIC website to get the Upfront medical exam and obtain the IMM 1017B form.

Description in the site says:

You require a medical exam. Learn how to get an upfront medical exam. In order to submit your online application, you will need to upload the information printout sheet or the IMM 1017B Upfront Medical Report form. Your doctor will give you one of these forms when you complete your medical exam.
If you cannot get a medical exam before the deadline to submit your work permit application, you may submit proof that you have a scheduled an appointment.

Doctor didn't ask for any reference number or anything like that, just inserted our passports info into eMedical system and performed the exams, blood tests and x ray test (after charging lots of money of course).

I just received the result file for one of my dependents (see below). My concern is if this is the right form. I don't know how IMM 1017B looks like but I don't think that's the IMM 1017B. Is that the correct form to upload into the system?

I got the same thing today. But Upfront Medical Type was "Worker". 501 Medical Examination said "Incomplete"
Right. Mine says "Worker" and "Incomplete" as well because doc still working on it.

The one from the image above is for one of my dependants, she's infant therefore didn't require additional tests so it's complete.

The fact you also received the same thing gives me some hope (unless both our doctors used the wrong form).

Yup, that's the IMM 1017B form. You'll have to scan and upload it - the key information here is your personal info + photo (for verification purposes) and the UMI number that links to the medical results.

For me, it said "Worker" and "Required". I passed the Medical Examination about two weeks later.

I got it in digital format, a PDF with text and the image. I'm inclined to just upload that pdf file instead of printing it and scanning later to convert to a single blob image. I'm doing the same for the police certificate, in my country it's done via their website, so the result is a PDF file with text in it. Please share your opinion if you see an issue with that. Thanks!

It shouldn't create any issues. Like I said, I think the main thing they're after is verifying your identity and obtaining the UMI-number from it, so as long as everything is legible it should be ok.

For PCC - does it have something like a signature or stamp on it? Is it literally just a white page with text on it? I would make sure CIC can actually verify it was issued by an authority, and not falsified (hence why PCCs usually are printed on special paper). Depending on what it looks like it may be best to make a mention of it in your LOE.

I would make sure CIC can actually verify it was issued by an authority, and not falsified (hence why PCCs usually are printed on special paper). Depending on what it looks like it may be best to make a mention of it in your LOE.
It has a number and a statement saying that the authenticity of the doc can be verified in the police's website with the number. Besides, when I go to this page and select my country, CIC seems to know that in here the police only issues PCC digitally (and has a link pointing me to the police website where I got mine).

You should be fine then. Still, it doesn't hurt to just remind them again they can verify the authenticity of the document through that website. Something like:

"Should there be any uncertainty regarding the authenticity of my Police Certificate, please follow this link to the website of the authority that issued the certificate and enter the reference number mentioned on the Certificate: <link>"
