He needs to fill the online form for the request - if you go through the steps, it pretty easy

However, they are still accepting old style of request till May 13, you can see my signature as I have made screenshots on how to fill with links

My husband was trying to pay by https://atip-aiprp.apps.gc.ca/atip/..but they won't accept ....he tried by 3-4.. cards but the system did not proced further. :-[....

My hubby did his medical in February of this year, and from what I understand, in the GCMS notes, under "Valid Until", it should have February 2014, which would be 1 year from - but it is extended to July 11, 2014! Should I take this as bad? Why are they extending it?
FYI: His medical was passed on July 11, 2013. It says No surveillance required. No health impairment.....

He was contacted with only a three-week notice for an interview....I ordered second set of GCMS notes and asked them if they can send them fast because of interview, but I do not think they will...:(

My notes also say: NO CSCD entered. Photos without info on the back. Case will be referred.

"RFV" = "Ready for Visa"
"PPT" = "Passport"

This means "please send a Passport Request, this file is ready to issue the COPR"
 
Is there any standard as to how many days after the information request is sent that the notes are pulled? From what I've read here it seems to vary; I've also read that recently (in the past three months or so) on average notes are generated/pulled 20 days after the GCMS request is sent and then emailed out by 30 days. Any thoughts?

Registry is where your file first goes when it gets to the VO, FC=Family Class the ST03055 is the officers number that put your information into the computer and this will change as your file goes to different officers.

hi dear forum members if we select physical file does it interfere processing time?
in order to get the working on file what should i check electronic?
because last time i check physical and they send me a cd in which there were random 188 scaned pages which i send them since i filed the case and nothing else like not the pattern which you guys are getting like creminality check: medical: bala bala ..
for this kind of notes what should i check?
and does ordering notes again and again effect the timeline or impact on case? after how long shoul i order notes again?

^ just select electronic file for GCMS, it will be sufficient.
Electronic request does not have any negative affect on processing. Takes about 30 days to receive after request.

It can. It requires someone in the visa office pull the physical file and scan it. What this can mean is that it is placed at the end of the processing queue. The only time it's safe in such cases is when there is already a scheduled event (e.g., an interview).

The electronic case notes should have everything that you want/need.
