You do not need to include any employer details, location, type of job, etc. It's in the name: Open Work Permit. Second question is answered by the same logic. Employer does not play a role, those options are there for restricted work permits. If you're still worried, in the explanation letter include:
- You're applying for OWP per Op. Bulletin 485
- A copy of your AOR letter
- A copy of your current work permit (they already have it but makes a nice package)
Your case is slightly different from mine in that you currently have a closed work permit. I'm not sure what to tell you there..
