where did you have your medical? ano po ba ibig sabihin ng DMP? pasensya na wala pa kac ako dyan na stage so hindi ako makarelate... hehehe...

Hello annjay...... had my medical sa cebu..... DMP means Designated Medical Practitioner..... sila yung mga acknowledged na Medical Practitioners of Canadian Embassy..... only thru them lang pwedeng magpa MR.....

yes as far as I know din we have two kinds of machine readable passports. One is the recently issued kind, with the hard dark maroon cover and the other one issued earlier (sometime 2008) na softer ang cover pero same color na dark maroon. Sandali lang sila nag issue ng passport na sabi machine readable pero soft cover. After months pinalitan na ng hard cover which is more believable to have a chip inside ;D

parehas tayo. yun ang information page sa passport na parang hologram na. my daughter has the epassport, and same kulay lang, same hitsura ng information page sa passport, pero makapal ang passport cover, tapos may parang ibang logo additional sa labas ng passport.

exactly, may parang hologram nga. so will this be still acceptable? acceptable siguro kasi pumasa ka sa application mo & MR ka na nga stage. right?

thanks for all the inputs at least I now have a peace of mind with regards to my passport.. I'm afraid I might get a new passport kac. buti na lang maroon yung passport ko hindi nga lang yung hard na cover pero at least acceptable pa rin since as you have said there were 2 kinds of machine readable passport. thanks forum mates.

guys, just a clarification lang... sa Schedule 1 background declaration form, sa Given names : 2 lines were provided for that portion... did you include your middle name there? i'm kinda paranoid on that portion kasi the once who helped me with my application advised me to place my middle name in the 2nd line... so in my correspondence regarding my application, like my PER & 2nd AOR...

i think ok lang. based ko lang to sa requirement din sa us embassy. last year kasi, kumuha ako ng visa sa us. requirement nila is yung machine readable. so far, wala naman silang sinabing problema sa passport ko.

I didn't put my middle name on the 2nd line. But nevertheless, on my PER they indicated my middle name just like yours.

application 1st received CIO AUG 16, 2010
submitted new IELTS result NOV 2010
received a letter from CIO that our application was returned AUG 26 2010 due to expired IELTS
after almost 4 months & thinking application was lost in transit..returned application delivered at our house dec 23, 2010 (ordinary mail-Barko pa talaga kaya 4 months bago dumating)
mailed same returned application, w/ new IELTS result DEC 24, 2010
application recieved CIO JAN 5, 2011
CC charged JAN 16, 2011
PER MARCH 11, 2011

thanks ladyk. that is one good option also. this makes sure the BD is referenced to the request from Embassy for payment.

ako kasi, wala PCC request, kasi nasama ko na sa pag submit sa CIO.

Saang bank ka pala nagpagawa ng bankdraft for Canadian Currency? and paano mo pinadala?

also, sa small envelope mo lang ba linagay? tapos finold mo nalang ang photocopy ng payment request?

Hi, doc! Sa BPI pa din, inilagay ko sa short brown envelope then pinacourier sa DHL.
