Mahesh I am also in process of collecting my docs... I am working wth a consultant... Yes you would have to get your Birth Certificate...
I also submitted PPF and PF statement...but that was returned by consultant... Submitting FD for the same amount

No ideas...Luckily I had one... You would have to contact local MCD around the hospital area for d details...
