Depends on where you are located. Some members mentioned a rate of $350 in NYC & VA. Mine was done for $415 in Philly. You can expect a range from $350 - $450 per adult.
I am not sure how much insurance will cover

For basic physical checkup my insurance charges me copay of 20... This insurance stuff is very confusing.......

SO wifey worked in Qatar for a while , Its best you get someone in Qatar to do it on your behalf . Just send them the documents listed in the link below . Also give them a letter of authority

http://www.cic.gc.ca/english/information/security/police-cert/mid-east/qatar.asp

It just takes a couple of days.

Lastly don't send to the USA through Aramark . Document got delivered to wrong address . I had applied and received a new one before Aramark could re-route the document .

I went for routine physical checkup 2 months back which included basic blood and urine test for cholesterol, sugar, blood pressure and other deficiencies in NYC and the bill was around 950$ but I was charged only 20$

I am not sure what tests are required for immigration and if they would be covered partially or fully in insurance

I can give exact details after I receive MR and do my tests....

Insurance most likely won't cover. I spent close to 1k USD [$] for me, spouse and our 4 year old.

That's difficult to say. FSW 2014 program received highest priority over other streams. Some of the files were processed faster than 2013 and even earlier application. EE is hot on the Government's agenda and they will likely fast-track it at the cost of delaying other applications.
Do you have any evidence to support you claims?

Insurance will not cover the expense. Insurance is paid based on medical codes, apparently there is no code that the physician can fill for immigration tests.
However, if you live in the US, you will be able to itemize the cost while filing your taxes.
