Congratulations on your PER!
To answer your first question, the next step is for CIC to send your application to your local VO. Your local VO will facilitate the request for a medical exam AND check also the authenticity/ accuracy of the documents included in your application kit (eg. the COEs, Bank Certificates, etc) and its completeness (eg. if you have submitted your PCC already or not). At this stage, your local VO will also conduct its background invesitigation/ security check. If there appears to be some points for clarification, the local VO will require you to attend an interview. If everything's clear, this step could be waived.
When the results of your medical, document and security checks are positive, you will now be asked to pay the RPRF and mail your passport to them for visa issuance.
PER to medical exam normally takes about 30-40 days from the threads I've read, but the timelines actually vary depending on the visa office processing the application. It will also depend on the accuracy and completeness of your docs and how fast you do your medicals. It goes without saying, the more complete and accurate your documents are, and the faster you act on additional requests by your VO, the faster you'll receive your visa.
