Sorry guys, posting it for 3rd time since morning, I hope someone has an answer to this.

In form IMM-5406 section A we are required to write the name of self, spouse, father and mother, and there is an adjoining column for 'Present Address' & 'Email Address', Whose address is to be written there. My parent and spouse lives in India and I live in UAE; I am confused because guideline too doesn't state whose address it should be.

We are expecting our baby, its been 3.5 months and possible delivery date is in first week of may'2015.

So now bit confuse that should we complete the medical(lets say we will do the x-ray too) and plan the delivery there or pause the medical and plan to land after our baby born here.

Medicals wont be considered complete without the chest x-ray. But chest x-rays pose no risk to the baby provided the abdomen area is covered with layers of lead apron. Let the X-ray technician know and he will ensure the use of lead apron and use of minimal power setting to avoid any exposure. Nowadays the x-ray beams are very well collimated and do not affect areas that are not being aimed at.

My wife got it done in 6th month, we did a lot of research and consulted a lot of doctors and decided to go ahead with the medicals. Sadly, the visa office has been sitting on medical results for almost 3 months now, the minimal risk we took seems to have rendered no benefit.

It is the address of the person you are indicating, this case, the address of your spouse and parents, not yours. please take note that, on the form for your spouse , she is the applicant.

Guys - useful info - it confirms they count app in CAP right after completeness (even before CC charge)
I got this info from their training manual which is available for public.

MailRoom

Processor #1 Service Delivery Clerk CR-03
Mail - Open mail, stamp envelope with received date, record tracking number on spreadsheet
enter daily mail count on G-drive spreadsheet,

Processor #2 - Service Delivery Clerk CR03:

Completeness Check - Review application for completeness <xyz...xyz>.
Ensure the application includes an acceptable language test, and that the cap has not been reached.

If application is complete: app is placed in a file jacket and then sent to be created in GCMS
If Arranged employment - staple orange tag to the file
If French processing - staple pink tag to be processed by bilingual agent

If application is not complete: a letter identifying the missing requirements is sent with
applicable appendices and the entire application is returned to the client.
Note: All returned applications must also be created in GCMS, for tracking purpose

Processor #3 Service Delivery Clerk
Scanning - GCMS application is created. Only essential details (Name, NOC, etc) are entered in GMS
initially to ensure the file is counted into the appropriate cap.
The file is then sent to Mailroom Processor #4 for cost recovery

Processor #4 Service Delivery Clerk
Cost Recovery - review the file to determine the method of payment....

You used consultant's cc?
Sometimes consultants would be ignorant. Talk with them and ask them to check with bank properly.

so In principal applicant case, I will put the address where my parent and spouse are staying and in my spouse's case, she will put the address of her parent and me. Am i right?

U should mention the address of ur parents where they are residing, and same for ur spouse
