Agree. What im saying is you will furnish your family details and the same with your spouse. But, you dont need this form to be filled for parents. They very clearly state this form needs to be filled by only applicant, spouse and children more than 18 years age.

1) No. The only thing that needs to be in native language is the name It should be written in the designated space by hand after printing. So, only one form for one person with everything in English except where name in native language is requested.

2)The form filled by the spouse, if you are the principal applicant then follow this: spouse's name in the applicant field and your name in the spouse field. Rest of the information should be the spouse's.

Withdrawal limit is not important here and POS transaction limit is ok.

But you need to check international card swipe daily limit. that's more important. If it is less than Rs 100K (to be on safer side), pls get it increased from the bank.
