Your spouse will be applicant in this form, so write your spouse name in applicant and aks your spouse to sign it....

My education is from India and I did not submit the transcripts form. I did not have any issues with the form and my evaluation was completed successfully. Same case with my other friend as well. Not sure about your country of education but if it is India then the issue is not with the transcripts form.
 
hii, just some random thought/ suggestion,

If you have any friend/ relative in Canada.. have him make a DD and courier it to CIO riteaway with an Explanation letter (CC failed so accept DD instead & pass me ..blah blah, + quote UCI, if any)

what if name is too big to incorporate in the space left over, should I need to use additional page to write name in native language? One more thing, signature should be the initial of the person or name in full?

Thanks for the information. I am from India too. I'll call up WES and ask them to precisely inform what is the issue and won't hangup till they do.
 
Hi all,
I'm new to this interesting forum. I just sent my app package & it should be received at the CIC office today! (Wed 04 Jun 14)

1- Could you please advise the timeline I should be expecting? (e-mail from CIC, CC charge, request for medical, etc..)

2- I will be moving to another country within few weeks as a permanent temporary resident for 2 years. When should I inform CIC of this change?

3- I will have a baby on Sep-14, when should I inform CIC & would that affect my app processing time?

For those whose CC has been charged, can you tell the forum whats the vendor name on the charge? Is it Citizenship Immigration Canada or Receiver General of Canada or CIC or some other?

It may help folks to reach out to their issuing banks and ensure that vendor is in safe list. May help reduce instances of CC declines due to fraud algorithms that sometimes will decline based on a large new foreign currency transaction.
