In my passport SURNAME field is empty, whole name has been entered in GIVEN NAME field,

so i should be entering my FULL NAME in Given name field right, and leave family name blank>??

Correct if i am doing it wrong.

And in FAMILY INFO, i am using all ENGLISH lang. no hindi as such ?

I believe you are from India.
You need to fill your form in English. There is a requirement in 2 sections, to fill your name in Native language. If married, then your spouse name in Native language.
If you are referring to "4.New Generic Form- IMM 0008 (03-2014)E",
Page 1: in Applications Details the 5th question. That is for Interview language, then keep it "English".
Page 3: In Language Details, Native Language selection, then you have an option to select Hindi, if your native language is Hindi
Just fill that section in Hindi.

Instructions fo filling up the form are pretty clear, they need your information as per your passport/ travel document.

If your passport doesnt have any family name then you should write Not Applicable or NONE or NA.

Your additional family information form requires all your names to be written in english and your native language, if you are intentionally leaving out the info then its at your own risk.

To give yourself a peace of mind, just write down their names in your native language atleast with a pen.
