Is it mandatory to write "I CERTIFY THAT THIS IS A TRUE COPY OF THE ORIGINAL DOCUMENT" on all the photocopies. Also
do we have to get all the photocopies like educational and work ref letter notarised.

Transcripts is needed.

Yes, it will do,I submitted Transcripts. Just check WES website for documents required for your Country and college.

but don't forget to send form you downloaded from WES along with Transcripts.

No need of writing such on the photocopies.......it wasn't stated .
You do not have to notorize them if the original from which they were photocopied ........if the original is not in English and was translated,it is necessary to notorize them.

You request for transcript for all the attempts you have given including the attempt in which you have failed. There is no specific format for Transcript. Apart from transcript you have to get photocopies of your all marksheets attested by your University. these both required for WES.

This is what WES say for India.
All individual mark sheets indicating all subjects taken and marks obtained for all years of post-secondary study – issued by the institution responsible for conducting the examinations.
e.g. university or autonomous college

NOTES:
College-issued and attested mark sheets will NOT be accepted unless from an autonomous college. To check whether your college has autonomous status click here.


Therefore transcripts are no problem

For most academic institutions ,transcripts and mark sheets are the same.
You are okay with transcripts ........reasons why I'm sure,that was what i used and it was ok.
Alternatively ,check the documents required from your country on WES website.

Hi,

In case employer is not giving Reference letter with Job duties, can I take a reference letter from my colleague on a plain paper with his contact details, who is at same or level than me. Taking letter from a Manager / Supervisor would be ideal but it could be damage the internal growth prospects.

It's a federal program so you are not restricted to live in any province choosen on your generic form.
There will be no problem for both of your requests .
