Just write "Parminder Kaur" in "Family Name" & leave "Given Name" blank (no need to write N/A). BTW, someone asked which one did I order, I ordered a Vegetarian Hot Pizza, from Domino's UK (I know they don't accept online / CC payment for delivery orders in India, but they do here in the UK)! BTW, Domino's pizza in the UK are much better compared to Domino's India or Domino's USA!

Actually, it's a funny story. I appeared for my IELTS back in 2009 (when I used to live in Mumbai), I scored 8 points. After living in London (and in an English speaking Country) for over 4 years, I thought I would manage to score 8.5 without any kind of prep so I decided to book test on earliest appointment available, which didn't allow me time to prepare for the test! I ended up with only 7.5 score, fortunately that was enough for the purpose of my application for FSW Program. They do accept online payment for delivery orders in India.
