My application was received by D. Macuray. I have not found anyone other than me whose application is received by D. Macuray (its a single "r"). I sent the application by Fedex and was delivered to "47 dorchester, Sydney, NS B1P5" although my destination address was "49 Dorchester Street". Is it only me guys or anyone also fetch the same issue. I have already communicated with Fedex to know the update. Anybode has any suggestion/advise please..

Thanks. I will keep everyone on this forum posted about my application. My app reached Sydney on May 28 and follow-up letter for payment changes reached Sydney on June 5. So, it will take some time before I come to know about the fate of my application..

Hello Guys,

If anyone has take a medical exam from USA and sent their package, please help me by providing the list of medical related documents you have included in your package.

I have a question on the list of medical documents to include in the package after the medical exam. As per the medical instructions, doctor will give you a document confirming that you underwent a medical exam. Also, if the doctor works with emedical they will give us up-front medical notification. Do we also need to include IMM1017 sent by CIC for me, my wife and my son? As per my understand, we need to include following in the package after the medical exam:

1. Doctor will give you a document confirming that you underwent a medical exam
2. For e-medical up-front medical notification
3. Not sure about IMM1017?

Is there any other medical related document to be included?

I took the medical and the physician is listed in the medical and waiting for my FBI clearance.. Please read the link http://www.cic.gc.ca/english/information/medical/medexams-perm.asp, the physician will directly send the report to CIC. We do not need to send them anything and the doctor did not provide me anything except the receipt. They took my picture and told me when they would get my report they would electronically submit to CIC. I think you are getting it mixed with the family class applicants( up front medical examination)where they have to attach the documents with the applications.

I need to get my BC translated, and also need to get an affidavit prepared specifying that my name is not on it. I know the latter can be done by the notary folks.

In India there are lot of people in Tehsil (where you register your property) does translation work. Some Notary also engaged in this activity. Go to Tehsil / Sub Divisional megistrate office and ask.

I have done it through notary people, but they are not providing any affidavit that he did this translation. They put one stamp and signed on translated document that i have done this translation from________ language to English.
