Hi Sumit,
1) If you are the primary applicant, you dont need to send the evaluated transcripts or even original transcripts or any work experience related documents at all.
2) If your ECA report shows evaluation of your B.E and MBA transcripts/Degrees that is enough. You do not need to send 10th and 12th marksheets.
3) High Gloss Best quality photograph. However there is a complete instruction sheet available on CIC regarding photo related specifications. Follow: http://www.cic.gc.ca/english/pdf/photospecs-e.pdf
4) Notarize any documents if they are an affidavit, self declaration of any sort or any other document which is not on official letterheads.
5) It is safer to get an affidavit done for place and date of birth and then get it notarized. As a reference you should also attach the existing document issued my Police. REMEMBER: Extra documents are never a problem, but lesser documents might be. And providing additional proofs will have CIC be confident of your authenticity.
6) The bank letter should ideally have the total fund mentioned in local currency and within brackets should have the total currency in CAD with conversion rate of a certain date)
