As you have got AOR so now you can send copy of updatedpassport.
No notorised copy required, but if you send notorised then that's fine too.
You need not to send any forms for this.
Just copy of passport and one simple self explanatory letter on plain paper where mention you file number and details of changes in your file and proof which you are attaching to it.
